Important Note for Authors

Before filling this FORM, Clinician shall take each patient’s consent duly signed after downloading from the APJNT website. Each consent form duly signed by each patient should be filed for record purposes at the Clinic.

After taking consent, Clinicians should fill-in data online for each patient
(Minimum 1 page, maximum 2 pages).
